A report of an investigation which was conducted by the Health and Information Quality Authority was published this morning and raises issue with Children and Family Agency, Tusla.

The report claims that children in need of assistance were placed at significant risk by the agency who, at times, failed to allocate a social worker when necessary.

In addition to this, HIQA asserts that allegations of abuse were not properly dealt with and Tusla failed to establish the risks children were exposed to regarding the accused.

The investigation into child protection and welfare services in Co. Cork was conducted last October.
