An investigation is due to launched following the tragic death of a 36-year-old mother in St. Luke's Hospital in Kilkenny on Easter Monday.

In a devastating turn of events, Tracey Fitzpatrick, a mother of three, died just minutes after giving birth to a healthy baby boy on Monday this week.

According to the Irish Mirror, the Co.Mayo-native, who lived in Carlow with her family, passed away as she was being taken into theatre for surgery following the safe delivery of her youngest child.

Commenting on Tracey's untimely passing, a spokesperson for St. Luke's Hospital said: "The staff and management of St Luke's Hospital in Kilkenny are currently focussed on supporting the family at this very sad time."

Confirming that an investigation is due to be launched into the circumstances surrounding Tracey's death, the spokesperson asserted: "An internal review process has commenced with a full investigation to follow."

Tracey, who married her husband Bernard just 17 months ago, was also mother to two older boys.
